- Registration Fee: A non-refundable registration fee reserves your spot
The Health and Admission Information Form will be collected at Open House before school starts:
- Immunizations – shot records must be current.
- Health Care Statement – A signed statement by your child’s physician is required.
- Every child four (4) years of age or older is required to have a vision and hearing screening yearly. This can be done by your child’s physician. We must have a current record of this screening for your child on file.
